Step into a pivotal role providing exceptional home health care within the community. This full-time, 13-week contract position offers the opportunity to make a genuine difference in patients lives by delivering skilled nursing care directly in their homes.

Schedule/Hours:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments and craft individualized care plans for each patient.
• Deliver skilled nursing interventions, including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, flag changes in health status, and communicate updates to the broader care team and physicians.
• Educate patients and families on health conditions, treatments, medications, and strategies for prevention.
• Accurately document all care using the Home Care Home Base (HCHB) electronic system.
• Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides to provide coordinated care.
• Participate in case conferences, team meetings, and ongoing continuing education.
• Maintain compliance with regulatory and agency standards for documentation and care delivery.
• Travel daily to patient residences within the assigned region, with mileage reimbursement provided.

Qualifications:

• Active and unrestricted RN license for the state.
• At least one year of recent field experience in Home Health nursing.
• Proficiency with Home Care Home Base (HCHB) and OASIS assessments.
• Reliable vehicle, valid drivers license, and current auto insurance.

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
