Deliver skilled and compassionate care to patients in the comfort of their own homes, supporting their health and independence during a 13-week full-time contract assignment. This role is ideal for nurses with home health experience who are confident with OASIS assessment, thrive working independently, and enjoy meaningful patient interactions in a field-based setting.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct thorough OASIS assessments and develop tailored care plans for patients.
• Deliver high-quality nursing care, including wound care, medication administration, IV therapy, and management of chronic illnesses during home visits.
• Monitor patient progress, assess for changes, and promptly update care teams and physicians as needed.
• Educate patients and their families on health conditions, treatment plans, medication management, and strategies for prevention.
• Complete accurate and timely clinical documentation using Home Care Home Base (HCHB).
• Collaborate closely with physical and occupational therapists, social workers, and home health aides to ensure coordinated and comprehensive care.
• Participate in regular case conferences, staff meetings, and continuing education.
• Maintain compliance with agency and regulatory standards in all documentation and care delivery.

Qualifications:

• Active, unencumbered RN license in the state of practice
• Minimum of 1 year recent Home Health experience required (field experience highly valued)
• Proficient in Home Care Home Base (HCHB) and OASIS assessments
• Reliable transportation, valid drivers license, and auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
