Your next nursing opportunity starts here. We are actively seeking an experienced Registered Nurse (RN) with expertise in home health and OASIS documentation for a 13-week full-time contract. This position provides a meaningful chance to make a difference in patients lives, working within a structured, supportive care team.

Schedule:

• Full-time commitment, Monday through Friday
• On-call responsibilities may be required
• 13-week contract duration with extension possibility

Key Responsibilities:

• Deliver in-home skilled nursing care and clinical interventions
• Complete accurate, timely OASIS assessments and documentation
• Execute wound care, medication management, disease monitoring, and patient/family education
• Collaborate with physicians and fellow care team members to ensure high-quality, individualized care plans
• Communicate patient status updates, changes in condition, and critical information promptly to the clinical team

Qualifications:

• Active RN license in good standing
• At least 1 year of recent home health RN experience required
• Demonstrated OASIS proficiency (required)
• Experience with HomeCare HomeBase preferred
• Strong organizational skills to balance daily patient visits and documentation
• Dependable transportation and valid drivers license
