Your next nursing opportunity starts with a rewarding, full-time contract for a Registered Nurse experienced in home health and OASIS documentation. This 13-week assignment offers the chance to truly make a difference, delivering skilled care one-on-one in patients homes, and being a vital part of collaborative care teams.

Schedule:

• Full-time, Monday through Friday
• On-call requirements likely
• Structured daily visit expectations

Key Responsibilities:

• Deliver compassionate, in-home skilled nursing care and interventions
• Perform comprehensive patient assessments and complete OASIS documentation thoroughly and on time
• Collaborate effectively with physicians and interdisciplinary care teams to create and maintain individualized care plans
• Provide wound care, medication management, patient and caregiver education, and chronic disease monitoring
• Clearly communicate any updates or changes in patient status to the broader clinical team

Desired Qualifications:

• Active RN license
• At least 1 year of recent home health RN experience
• OASIS documentation expertise required
• Strong proficiency with HomeCare HomeBase preferred
• Capable of managing a structured, high-volume schedule with consistent daily patient visits
• Reliable transportation and valid drivers license
