Job Description

Take on a vital role delivering skilled nursing care in a home health setting. This full-time, 13-week contract opportunity is designed for experienced RNs with a strong background in home health. The position involves field-based patient care and requires comfort with a flexible, travel-oriented schedule.

Schedule:

  • Monday to Friday, 8:00 AM 5:00 PM

Key Responsibilities:

  • Perform comprehensive OASIS assessments and develop tailored care plans.
  • Administer skilled nursing interventions such as wound care, medication management, IV therapy, and chronic disease oversight.
  • Monitor patient progress, recognize changes in condition, and communicate promptly with the healthcare team and physicians.
  • Provide education to patients and families regarding disease processes, medications, treatments, and health maintenance.
  • Accurately and efficiently complete electronic documentation using Home Care Home Base (HCHB).
  • Coordinate with interdisciplinary team members, including therapists, social workers, and aides.
  • Attend case conferences, staff meetings, and participate in ongoing training as needed.
  • Comply with all regulatory and agency standards for documentation and patient care delivery.

Qualifications:

  • Active RN license in good standing for the state of practice
  • At least 1 year of recent home health experience (field-based preferred)
  • Proficiency in Home Care Home Base (HCHB) and OASIS required
  • Reliable vehicle, valid drivers license, and auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
