Step into a dynamic, patient-focused home health nursing role, providing essential care within your community. This full-time, field-based position offers a 13-week contract where you'll be pivotal in supporting patients in their own homes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Qualifications:
- Active, unencumbered RN license in the state of practice
- At least 1 year of recent home health nursing experience (field experience strongly preferred)
- Skilled in using Home Care Home Base (HCHB) for electronic documentation
- Previous OASIS assessment experience
- Reliable transportation, with valid drivers license and auto insurance
Essential Responsibilities:
- Conduct comprehensive OASIS assessments and develop patient-specific care plans
- Perform skilled nursing interventions, including wound care, medication administration, IV therapy, and chronic disease management
- Monitor patient progress, identify any changes in condition, and communicate effectively with the wider care team and physicians
- Educate patients and their families on disease management, medications, and preventative care
- Ensure timely, accurate documentation within Home Care Home Base (HCHB)
- Collaborate closely with interdisciplinary team members such as therapists, medical social workers, and home health aides
- Participate in case conferences, staff meetings, and continuing education to maintain quality standards
- Adhere to all clinical and regulatory guidelines for home health care

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
