This 13-week, full-time contract position is an excellent opportunity for skilled nurses who thrive outside the traditional clinical setting and are confident managing a diverse caseload in patient homes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct OASIS assessments and develop individualized patient care plans.
• Deliver skilled nursing services such as wound care, IV therapy, medication administration, and chronic disease management.
• Assess and monitor patient conditions, reporting changes promptly to physicians and care teams.
• Provide education to patients and families about disease management, medication compliance, and preventive care.
• Collaborate with interdisciplinary staff including therapists, social workers, and aides for seamless patient care coordination.
• Ensure accurate, timely electronic documentation in Home Care Home Base (HCHB).
• Attend case conferences, staff meetings, and professional development sessions to support ongoing excellence.
• Adhere rigorously to regulatory and best-practice standards in all patient care and documentation.

Qualifications:

• Current, active RN license in good standing for the state of practice.
• At least one year of recent field-based Home Health nursing experience.
• Proficiency with OASIS assessment and the Home Care Home Base (HCHB) electronic documentation system.
• Reliable transportation, a valid drivers license, and active auto insurancedaily territory travel is required.

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
