We are seeking a Registered Nurse (RN) with strong Home Health and OASIS documentation skills for a full-time, 13-week contract opportunity. This position offers a structured schedule, professional collaboration, and a chance to provide personalized care where it matters most.
Schedule:
- Full-time, Monday through Friday
- On-call responsibilities may be required
- Daily visit expectations based on caseload
Desired Qualifications:
- Active RN license
- Minimum of 1 year recent experience as a home health RN
- Proficiency with OASIS documentation is required
- Experience with HomeCare HomeBase preferred
- Demonstrated ability to handle a structured schedule with daily patient visits
- Reliable transportation and a valid drivers license
Primary Responsibilities:
- Deliver skilled nursing interventions and compassionate in-home care
- Conduct patient assessments and complete OASIS documentation accurately and promptly
- Coordinate care with physicians and multidisciplinary teams to ensure optimal outcomes
- Perform wound care, medication management, patient and family education, and chronic disease monitoring
- Provide timely communication of patient status or changes to the broader care team
