Schedule:
- 13-week contract, strong potential for extension
- Full-time, Monday through Friday
- On-call duties may be required
Qualifications:
- Current, active RN license
- At least 1 year of recent home health experience as an RN
- Proficient in OASIS documentation
- Experience with HomeCare HomeBase is strongly preferred
- Dependable transportation and valid drivers license
- Ability to maintain a structured, consistent patient visit schedule
Responsibilities:
- Deliver skilled nursing care and interventions to patients in their homes
- Conduct comprehensive patient assessments; complete OASIS documentation promptly and accurately
- Collaborate closely with physicians and interdisciplinary care teams to tailor individualized care plans
- Perform wound care, medication management, patient education, and ongoing disease monitoring
- Clearly communicate patient status and updates with the clinical team to ensure high-quality care
