Job Description
Embark on a meaningful assignment as a Registered Nurse specializing in Home Health care. This contract position offers the vital opportunity to deliver compassionate support to patients needing skilled care in their homes. With your home health and travel experience, you'll thrive in a dynamic environment that values both clinical skill and autonomy.
Schedule:
- Monday to Friday, 8:00 am to 5:00 pm
- 5x8-hour day shifts
- Assignment duration: 8 weeks, with possibility for extension up to 12 weeks
Key Qualifications:
- Active RN license
- Minimum 1 year of case management experience
- Demonstrated background in Home Health settings (HCHB and travel experience required)
- Self-motivated with strong organizational skills
- Comfortable managing a personal caseload and working independently
- Must possess a personal vehicle and be willing to drive throughout the Greensboro and High Point areas
Primary Responsibilities:
- Provide comprehensive nursing care to home health patients as assigned
- Conduct patient assessments and develop, implement, and monitor individualized care plans
- Manage a full caseload, ensuring timely visits and thorough documentation
- Collaborate with interdisciplinary teams to coordinate holistic patient care
- Educate patients and families on treatment plans, disease management, and preventative practices
- Maintain accurate and timely clinical records in accordance with agency and regulatory standards
