Job Description
A Registered Nurse with home health and travel experience is needed for an 8-week contract opportunity in the Kernersville, NC area, with potential extension to 12 weeks based on census. This role requires case management of a full caseload, providing support to patients throughout the Greensboro and High Point regions.
Key Responsibilities:
- Manage and coordinate home health nursing care for assigned patients
- Conduct comprehensive patient assessments in home settings
- Develop and implement individualized care plans
- Collaborate with interdisciplinary teams to ensure continuity of care
- Maintain accurate documentation and communicate effectively with healthcare providers and patients
- Travel throughout the assigned area a reliable personal vehicle is required
Qualifications:
- Current Registered Nurse (RN) license
- Minimum 1 year of case management experience in home health nursing
- Previous travel nursing experience preferred
- Strong organizational and communication skills
- Ability to work independently and manage multiple cases efficiently
Schedule:
- Monday to Friday, 8:00 AM to 5:00 PM (five 8-hour shifts)
Location Details: Kernersville, NC, is centrally located in the Piedmont Triad region, offering easy access to Greensboro and High Point. Additional Information:
- Mileage reimbursement provided at the federal rate
- This position requires daily travel; candidates must have their own vehicle
- This is not an inpatient role
