Your next nursing opportunity starts herea full-time, 13-week home health contract for a Registered Nurse with OASIS experience. This position offers the chance to deliver compassionate, skilled nursing care in a home setting, while working with a supportive and professional care team.
Schedule:
- Full-time, Monday through Friday
- On-call responsibilities may be required
- Structured daily visit schedule
Required Qualifications:
- Active RN license
- At least 1 year of recent home health RN experience
- Proficient with OASIS documentation
- Experience with HomeCare HomeBase strongly preferred
- Reliable transportation and a valid driver's license
- Ability to manage a consistent daily caseload
Key Responsibilities:
- Deliver in-home nursing care, skilled clinical interventions, and patient education with a focus on individualized treatment plans
- Complete thorough patient assessments, ensure OASIS documentation accuracy, and submit records promptly
- Collaborate with physicians and interdisciplinary care teams to optimize patient outcomes
- Manage wound care, medication administration, and chronic disease monitoring
- Communicate changes in patient condition and progress updates to team members
