A dedicated Registered Nurse (RN) with OASIS experience is needed for a rewarding 13-week full-time home health contract in Kenner, LA. This role involves delivering skilled nursing care in patients homes, managing clinical documentation, and collaborating closely with healthcare teams to provide excellent patient-centered care.
Key Responsibilities:
- Provide comprehensive in-home nursing care and skilled interventions
- Perform thorough patient assessments and complete OASIS documentation accurately and promptly
- Collaborate with physicians and interdisciplinary care teams to implement individualized care plans
- Administer wound care, medication management, patient education, and disease monitoring
- Communicate patient status updates and any condition changes to the clinical team
Qualifications:
- Active Registered Nurse (RN) license
- Minimum of 1 year recent home health nursing experience
- Required experience with OASIS documentation
- Proficiency in HomeCare HomeBase is strongly preferred
- Ability to manage a structured full-time schedule and complete daily patient visits
- Reliable transportation and a valid drivers license
Schedule:
- Full-time, Monday through Friday
- On-call availability as needed
