Schedule/Hours:

• Full-time, 13-week assignment
• Monday Friday, 8:00 AM 5:00 PM
• Daily field-based travel to patient homes within your assigned territory

Key Responsibilities:

• Perform comprehensive OASIS assessments and develop personalized care plans
• Deliver skilled nursing care including wound management, medication administration, IV therapy, and chronic disease monitoring
• Monitor patient progress, identify condition changes, and consult with the broader care team and physicians as needed
• Educate patients and family members regarding diagnoses, medication regimens, and health improvement strategies
• Maintain timely, accurate documentation in Home Care Home Base (HCHB)
• Collaborate closely with physical and occupational therapists, social workers, and home health aides
• Participate actively in case conferences, staff meetings, and ongoing educational opportunities
• Uphold all regulatory and agency standards for documentation and care delivery

Desired Experience/Qualifications:

• Current RN license in good standing
• Minimum 1 year of recent home health experience (field-based strongly preferred)
• Proficiency using Home Care Home Base (HCHB)
• Solid background with OASIS assessments
• Dependable transportation, valid drivers license, and active auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
