Your next opportunity to make a lasting impact starts here. We are seeking a passionate Registered Nurse with recent home health experience for a 13-week, full-time contract role. If you have strong clinical skills, a keen eye for documentation, and thrive in a structured environment, youll find this position both rewarding and meaningful.
Schedule:
- Full-time, Monday through Friday
- Daily patient visit expectations
- On-call required as needed
Key Responsibilities:
- Deliver skilled nursing care to patients in their homes, including wound care, medication management, and patient education
- Perform comprehensive patient assessments and ensure timely, accurate OASIS documentation
- Collaborate effectively with physicians and interdisciplinary care teams to craft and adjust individualized care plans
- Monitor conditions, communicate changes promptly, and maintain detailed clinical notes
- Provide education and support that empowers patients and their families
Qualifications:
- Active RN license in good standing
- At least 1 year of recent experience as a home health RN
- Proven proficiency with OASIS documentation (required)
- Experience with HomeCare HomeBase (strongly preferred)
- Ability to independently manage a structured, full-time visit schedule
- Reliable vehicle and current drivers license
