An exciting contract opportunity is available for an experienced Registered Nurse specializing in home health care. This role focuses on delivering compassionate, high-quality nursing services to patients in the comfort of their own homes. Schedule/Hours:
- Full-time contract role; specific hours to be discussed during the interview process.
Qualifications:
- Active, unrestricted RN license.
- Minimum of 3 months home health nursing experience with OASIS documentation required.
- Experience with HCHB (Homecare Homebase) is highly desirable.
- Proficiency with electronic medical records (EMR).
- Strong clinical expertise, including IV therapy, wound care, and medication management.
- Excellent interpersonal and communication abilities.
- Ability to work independently, prioritize tasks, and manage time effectively.
- Valid drivers license and access to reliable transportation.
Key Responsibilities:
- Deliver comprehensive nursing care to patients in a home setting.
- Conduct initial and follow-up patient assessments: vital signs, wound care, medication administration.
- Develop and implement patient-specific care plans.
- Educate patients and families on disease processes, treatment protocols, and preventive care.
- Collaborate closely with physicians, therapists, and other healthcare professionals.
- Accurately document care activities using EMR systems.
- Monitor and promptly report changes in patient status to overseeing physicians.
- Ensure alignment with all relevant state and federal home health regulations.
