We are seeking a dedicated Registered Nurse with strong experience in home health to work a full-time, 13-week contract. This position will allow you to apply your clinical skills and OASIS expertise while making a meaningful difference in the lives of patients receiving care in their own homes. You will be part of a collaborative and professional environment that values patient-centered outcomes and genuine compassion.
Schedule:
- 13-week contract with the possibility of extension
- Full-time, Monday through Friday
- On-call duties may be required
Key Responsibilities:
- Deliver skilled nursing care and interventions in patient homes
- Accurately complete OASIS assessments and documentation in a timely manner
- Collaborate with physicians and other healthcare professionals to develop and implement individualized care plans
- Provide wound care, oversee medication management, educate patients and families, and monitor chronic conditions
- Communicate patient status updates and any changes in condition to the broader clinical team
Qualifications:
- Active RN license in good standing
- Minimum of 1 year of recent experience as a home health RN
- Proven OASIS assessment and documentation skills
- Proficiency with HomeCare HomeBase is strongly preferred
- Ability to manage a full-time schedule and meet daily visit expectations
- Reliable transportation and a valid drivers license
