We are seeking a dedicated Registered Nurse with proven home health experience, including OASIS documentation proficiency, for a full-time, 13-week contract assignment. This role is ideal for RNs confident in clinical skills, detail-oriented with documentation, and comfortable managing a structured schedulemaking a real difference in patients lives in the comfort of their own homes.
Schedule:
- Full-time, Monday through Friday
- On-call duties may be assigned
- Daily visit expectations
Qualifications:
- Active RN license
- At least one year of recent home health RN experience
- Required OASIS assessment and documentation experience
- Familiarity with HomeCare HomeBase (strongly preferred)
- Reliable transportation and valid drivers license
- Strong time management and patient communication skills
Responsibilities:
- Deliver skilled nursing care in patients homes, including assessments, interventions, and education
- Complete accurate OASIS documentation in a timely manner
- Coordinate care and updates with physicians and the broader multidisciplinary team
- Manage wound care, administer medications, monitor disease processes, and educate patients and families
- Report changes in patient conditions promptly to the clinical team
