Advance your nursing career by delivering high-quality, patient-centered care in a field-based setting. This full-time, 13-week contract offers the opportunity to work with diverse individuals and families in their homes, making a meaningful impact in the community. You'll use your clinical expertise to provide comprehensive care, conduct OASIS assessments, formulate and implement care plans, and coordinate with interdisciplinary team members.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Perform thorough OASIS assessments and develop tailored patient care plans.
• Deliver skilled nursing interventions, such as wound care, IV therapy, medication administration, and chronic disease management.
• Monitor patient progress, note any changes in condition, and communicate with physicians and care teams as necessary.
• Offer patient and family education regarding treatment, disease management, medications, and preventive care measures.
• Maintain up-to-date records in the Home Care Home Base (HCHB) electronic documentation system.
• Coordinate care by collaborating with colleagues across disciplines, including physical and occupational therapists, social workers, and home health aides.
• Engage in case conferences, staff meetings, and continuing education.
• Uphold all regulatory and organizational standards in documentation and care delivery.
• Travel daily to patient homes within your assigned area (mileage reimbursement provided).

Qualifications:

• Current, active RN license (state of practice) in good standing.
• Minimum of 1 year recent home health experience; field experience highly preferred.
• Proficiency with HCHB and documented experience with OASIS assessments.
• Reliable transportation, a valid drivers license, and active auto insurance.

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
