Schedule:

Key Responsibilities:

  • Deliver skilled in-home nursing care, including wound management, medication administration, patient education, and disease monitoring
  • Complete precise and timely OASIS assessments and documentation
  • Partner with physicians and interdisciplinary teams to develop tailored care plans
  • Keep the clinical team apprised of patient progress and any status changes

Qualifications:

  • Active RN license
  • Minimum of 1 year of recent experience as a home health RN
  • Required proficiency in OASIS documentation
  • HomeCare HomeBase experience strongly preferred
  • Reliable transportation and valid drivers license
  • Ability to maintain a full-time, structured schedule with consistent daily patient visits

$2,193 - $2,312/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
