A full-time, 13-week opportunity is available for experienced Registered Nurses with a strong background in home health care. This field-based position provides skilled nursing services to patients in their own homes, ensuring high-quality and coordinated care.

Schedule & Work Structure:

• Monday Friday, 8:00 AM 5:00 PM
• Caseload expectation of 30 points per week (typically 35 patient visits per day)
• Daily travel to patient homes within assigned territory (mileage reimbursed)

Key Responsibilities:

• Complete comprehensive OASIS assessments and develop individualized care plans
• Deliver skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management
• Monitor patient progress, identify changes in condition, and communicate promptly with the interdisciplinary team and physicians
• Educate patients and their families about disease processes, treatment plans, and preventive care
• Maintain accurate, timely documentation using Home Care Home Base (HCHB)
• Collaborate with physical and occupational therapists, medical social workers, and home health aides for integrated patient care
• Participate in case conferences, staff meetings, and ongoing education
• Uphold regulatory and agency standards in care delivery and documentation

Qualifications:

• Current, active RN license in good standing for the state of practice
• At least 1 year of recent home health nursing experience, preferred field background
• Proficient with Home Care Home Base (HCHB) and OASIS
• Dependable transportation, valid drivers license, and active auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
