Deliver compassionate, patient-centered nursing care in a dynamic home health setting. This 13-week full-time contract position focuses on providing skilled nursing services directly in patients homes, utilizing your expertise to promote optimal patient outcomes and quality of life.

Schedule/Hours:

• Monday Friday, 8:00 AM 5:00 PM

Key Qualifications and Requirements:

• Current, active RN license in good standing for the state of practice
• At least one year of recent home health nursing experience (field experience strongly preferred)
• Proficiency with Home Care Home Base (HCHB) electronic documentation system
• Experience conducting and documenting OASIS assessments
• Reliable vehicle, valid drivers license, and up-to-date auto insurance

Role Responsibilities:

• Conduct thorough OASIS assessments and develop individualized patient care plans
• Deliver skilled nursing care in the field, including wound care, medication administration, IV therapy, and chronic disease management
• Monitor and communicate changes in patient condition to ensure appropriate care adjustments
• Educate patients and families on disease management, medications, and preventive care strategies
• Accurately document all patient encounters in HCHB system
• Collaborate with a multidisciplinary teamphysical therapists, occupational therapists, social workers, and aidesto coordinate comprehensive care
• Attend case conferences, staff meetings, and continuing education sessions as required
• Ensure compliance with all agency and regulatory standards for patient care and documentation
• Manage a caseload averaging 35 patient visits daily, with a weekly target of 30 points
• Travel daily within assigned territory (mileage reimbursement provided)

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
