We are seeking a dedicated Registered Nurse (RN) with OASIS expertise for a 13-week full-time home health contract. Schedule:
- 13-week contract with potential to extend
- Full-time, Monday through Friday
- On-call responsibilities as needed
Key Responsibilities:
- Deliver comprehensive in-home nursing care and skilled clinical interventions
- Conduct thorough patient assessments and ensure timely, accurate completion of OASIS documentation
- Develop, implement, and reassess individualized care plans in collaboration with physicians and interdisciplinary care teams
- Administer wound care, manage medications, provide patient and family education, and monitor disease progression
- Communicate promptly with the clinical team regarding patient updates or changes in condition
Required Qualifications:
- Active RN license
- At least 1 year of recent home health RN experience
- Demonstrated OASIS competency
- Strong proficiency with HomeCare HomeBase (highly preferred)
- Commitment to maintaining a structured daily visit schedule
- Reliable transportation and a valid drivers license
