Job Description

An opportunity is available for a Registered Nurse (RN) with specialized experience in home health nursing to provide comprehensive care to patients in a home setting. This contract role requires proficiency in OASIS assessments and a dedication to delivering high-quality patient care.

Key Responsibilities:

  • Conduct initial and ongoing patient assessments, including vital signs, medication administration, and wound care.
  • Develop and execute individualized care plans tailored to patient needs.
  • Educate patients and families on disease management, treatment protocols, and preventive care measures.
  • Collaborate closely with physicians, therapists, and multidisciplinary teams to ensure coordinated care.
  • Accurately document all nursing activities in electronic medical records (EMR).
  • Monitor patient condition changes and report promptly to the supervising physician.
  • Maintain compliance with state and federal home health care regulations.

Qualifications:

  • Active RN license.
  • Minimum of 3 months home health nursing experience using OASIS.
  • Experience with HomeCare HomeBase (HCHB) is highly desirable.
  • Strong clinical skills including IV therapy, wound care, and medication management.
  • Proficient in EMR documentation.
  • Excellent communication and interpersonal abilities.
  • Ability to work independently with effective time management.
  • Valid drivers license and reliable transportation required.
