Key Responsibilities:
- Deliver skilled nursing care during in-home visits, including wound care, medication management, patient education, and disease monitoring
- Conduct thorough patient assessments and complete OASIS documentation accurately and on schedule
- Collaborate closely with physicians and multidisciplinary care teams to implement personalized care plans
- Communicate promptly any changes in patient conditions or concerns to clinical teams
- Manage structured daily visit schedules efficiently
Qualifications:
- Active Registered Nurse (RN) license
- Minimum one year of recent home health nursing experience
- Proven proficiency with OASIS documentation is required
- Experience with HomeCare HomeBase electronic documentation system strongly preferred
- Reliable transportation and valid drivers license essential
- Ability to manage a structured full-time schedule Monday through Friday, with possible on-call responsibilities
