Take your home health nursing expertise into the field with an engaging full-time contract role specializing in patient-centered care. This 13-week assignment focuses on delivering quality clinical services directly in patient homes, ensuring seamless coordination and compassionate support.

Schedule & Setting:

• Monday through Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Field-based: Daily travel within an assigned territory; mileage reimbursement provided
• Typical caseload: 35 patient visits per day, aiming for 30 points per week

What Youll Do:

• Complete comprehensive OASIS assessments and develop individualized care plans
• Provide skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management
• Constantly evaluate patient status and communicate changes to physicians and multidisciplinary team members
• Offer education to patients and families on disease management, medication regimens, and prevention
• Maintain precise, timely documentation within Home Care Home Base (HCHB)
• Collaborate closely with physical therapists, occupational therapists, social workers, and home health aides
• Engage in ongoing training and participate in meetings to support best practices and continuous improvement
• Consistently follow regulatory and agency guidelines for safe and effective patient care

Qualifications:

• Active RN license (in good standing) in state of practice
• At least one year of recent Home Health experience (field experience highly preferred)
• Proficient with Home Care Home Base (HCHB)
• Direct experience completing OASIS assessments
• Reliable transportation, valid driver's license, current auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
