We are seeking an experienced Registered Nurse (RN) with a background in home health and proficiency in OASIS documentation for a 13-week full-time contract. Schedule:
- Full-time, Monday through Friday
- On-call duties may be required
- Structured daily visit expectations
Key Responsibilities:
- Deliver in-home nursing care and skilled nursing interventions
- Assess patients and complete OASIS documentation accurately and in a timely manner
- Collaborate closely with physicians and interdisciplinary care teams to devise and implement individualized care plans
- Perform wound care, manage medications, educate patients, and monitor disease processes
- Keep the clinical team informed with timely communication regarding patient updates or changes in condition
Qualifications:
- Current, active RN license
- At least one year of recent home health RN experience
- OASIS documentation experience is required
- Proficiency with HomeCare HomeBase is strongly preferred
- Ability to manage a consistent, full-time schedule with daily patient visits
- Reliable transportation and a valid drivers license
