Job Description
Deliver essential nursing care in a field-based setting, supporting patients in their homes on a full-time, 13-week assignment. Work Monday through Friday from 8:00 AM to 5:00 PM, managing a caseload of 30 points per week (typically 35 patient visits daily). Travel throughout the assigned territory is an integral part of this position, with mileage reimbursement provided.
Qualifications:
- Valid RN license in the state of practice, in good standing
- At least 1 year of recent home health nursing experience (field-based preferred)
- Proficient in Home Care Home Base (HCHB) electronic documentation
- Previous OASIS assessment experience required
- Reliable transportation, current drivers license, and auto insurance
Core Responsibilities:
- Conduct thorough OASIS assessments and craft individualized patient care plans
- Deliver skilled nursing interventions including wound care, medication administration, IV therapy, and chronic disease management
- Closely monitor patient progress, recognizing significant changes and updating the healthcare team appropriately
- Educate patients and their families regarding health conditions, medications, and preventive strategies
- Accurately document care in HCHB and meet all relevant regulatory requirements
- Collaborate with allied health professionals and participate in case conferences and staff meetings to ensure high-quality, coordinated patient care

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
