A rewarding contract position is available for an experienced Registered Nurse specializing in home health. Schedule:

• 13-week contract, with the possibility of extension
• Full-time, Monday through Friday
• On-call participation is probable
• Daily structured patient visits

Key Responsibilities:

• Deliver in-home skilled nursing care aligned with individualized treatment plans
• Conduct comprehensive patient assessments and accurately complete OASIS documentation on schedule
• Collaborate effectively with physicians and interdisciplinary care teams
• Manage wound care, administer medications, provide patient education, and monitor diseases
• Promptly communicate patient condition changes to the clinical team

Qualifications:

• Active RN license in good standing
• At least one year of recent home health RN experience
• Demonstrated experience with OASIS documentation (required)
• Familiarity with HomeCare HomeBase (strongly preferred)
• Capable of managing a consistent, high-volume visit schedule
• Reliable transportation and a valid drivers license
