Schedule:

• 13-week contract duration, full-time
• Monday through Friday shifts

Key Qualifications:

• Current RN license in good standing
• At least one year of recent home health RN experience
• Proficiency in OASIS documentation is mandatory
• Familiarity with HomeCare HomeBase (strongly preferred)
• Reliable transportation and a valid drivers license
• Strong organizational skills and the ability to manage daily patient visits

Primary Responsibilities:

• Deliver in-home nursing care and skilled interventions
• Assess patients and complete thorough, timely OASIS documentation
• Develop and implement individualized care plans through close collaboration with physicians and care teams
• Administer wound care, medication management, patient education, and chronic disease monitoring
• Communicate any patient condition changes promptly to the clinical team
