Job Description
An opportunity is available for a Registered Nurse (RN) with home health experience to provide compassionate and comprehensive nursing care to patients in their homes on a contract basis. This role requires expertise in OASIS assessments and a commitment to improving patient outcomes through individualized care plans.
Key Responsibilities:
- Deliver skilled nursing care, including IV therapy, wound care, medication management, and vital signs monitoring.
- Perform initial and ongoing patient assessments using OASIS guidelines.
- Develop, implement, and update individualized patient care plans.
- Educate patients and families on disease management, treatment strategies, and preventive measures.
- Collaborate with physicians, therapists, and healthcare professionals to maintain coordinated care.
- Maintain accurate and timely documentation of patient care in electronic medical records (EMR).
- Monitor patient conditions and report any changes to the supervising physician.
- Ensure adherence to state and federal home health care regulations.
Qualifications:
- Active RN license.
- Minimum of 3 months home health experience with proficiency in OASIS.
- Experience with Homecare Homebase (HCHB) is highly desirable.
- Strong clinical skills in IV therapy, wound care, and medication administration.
- Proficient with EMR systems.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Valid drivers license and reliable transportation.
