Advance your nursing career in a field-based role focused on providing compassionate care directly in patients homes. This full-time, 13-week contract offers a meaningful opportunity to deliver skilled nursing services while enjoying consistent weekday hours.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct thorough OASIS assessments and develop individualized care plans for each patient.
• Deliver skilled nursing interventions, such as wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient conditions, evaluate progress, and communicate changes to the interdisciplinary care team and physicians.
• Educate patients and their families regarding disease management, treatment regimens, and preventive health practices.
• Complete documentation accurately and promptly using Home Care Home Base (HCHB).
• Collaborate seamlessly with physical therapists, occupational therapists, medical social workers, and home health aides to coordinate whole-patient care.
• Participate in case conferences, staff meetings, and ongoing education.
• Ensure compliance with all regulatory and organizational standards relating to patient care and documentation.

Qualifications:

• Active RN license in good standing for the state of practice
• At least 1 year of recent experience in Home Health (field experience preferred)
• Proficiency with Home Care Home Base (HCHB)
• OASIS assessment experience required
• Reliable transportation, current drivers license, and valid auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
