Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Qualifications:

• Active, unencumbered RN license in the state of practice
• At least 1 year of recent home health nursing experience (field work strongly preferred)
• Proficiency with Home Care Home Base (HCHB) electronic documentation
• Demonstrated experience with OASIS assessments
• Reliable transportation, valid drivers license, and current auto insurance

Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans for each patient
• Deliver skilled nursing care, such as wound care, medication administration, IV therapy, and chronic disease management in patient homes
• Monitor and document patient progress, recognizing and promptly responding to changes in condition
• Educate patients and families regarding disease processes, medications, care plans, and preventive strategies
• Accurately and promptly document all visits and interventions in the HCHB system
• Foster interdisciplinary collaboration by working with therapists, aides, social workers, and other healthcare professionals
• Attend case conferences, participate in staff meetings, and engage in continuing education as needed
• Uphold all regulatory and best-practice standards for patient care

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
