Your next nursing opportunity starts here. We are seeking a dedicated Registered Nurse with strong OASIS experience for a full-time, 13-week contract in home health. If you excel at clinical care, precise documentation, and managing a structured schedule, youll thrive in this impactful role.
Schedule:
- Full-time, Monday through Friday
- On-call responsibilities possible
- Structured daily visit expectations
What to Expect:
- 13-week contract, with possibility for extension
- Opportunity to partner with a collaborative, professional care team
Key Responsibilities:
- Deliver skilled in-home nursing care and interventions
- Perform thorough patient assessments and complete all OASIS documentation accurately and promptly
- Work closely with physicians and the care team to implement tailored patient care plans
- Provide wound care, medication management, patient teaching, and monitoring of chronic or acute conditions
- Communicate any changes in patient status to the clinical team
Qualifications:
- Active RN license
- At least 1 year of recent home health RN experience
- OASIS documentation experience required
- Familiarity with HomeCare HomeBase (highly preferred)
- Ability to maintain a consistent, full-time home health schedule
- Reliable transportation and a valid drivers license
