If you excel in OASIS documentation, autonomous clinical practice, and thrive in a fast-paced environment delivering personalized care to patients in their homes, this full-time contract is designed for you.

Schedule:

  • 13-week contract, with potential for extension
  • Full-time, Monday through Friday
  • Daily home health visits and possible on-call requirements

Key Responsibilities:

  • Deliver in-home nursing care and skilled interventions with professionalism and empathy
  • Complete thorough patient assessments and ensure timely, accurate OASIS documentation
  • Collaborate with physicians and interdisciplinary teams to develop and adjust comprehensive care plans
  • Perform wound care, medication administration, disease process monitoring, and patient/family education
  • Maintain consistent, clear communication regarding patient condition updates with the clinical team

Qualifications:

  • Active RN license (unrestricted)
  • At least 1 year of recent home health nursing experience
  • Required OASIS documentation expertise
  • Familiarity with HomeCare HomeBase preferred
  • Capacity to manage a structured daily schedule with multiple patient visits
  • Reliable transportation and valid drivers license
