A leading healthcare employer is seeking an experienced Registered Nurse (RN) specializing in home health for a 13-week full-time contract. This role offers the chance to utilize advanced clinical skills and provide compassionate care for patients in their homes. Schedule:
- Full-time, Monday through Friday
- On-call rotation may be required
Key Responsibilities:
- Deliver skilled in-home nursing care and interventions to a diverse patient population
- Perform thorough patient assessments and complete OASIS documentation accurately and in a timely manner
- Collaborate closely with physicians and multidisciplinary teams to implement tailored care plans
- Provide wound care, medication management, patient education, and chronic disease monitoring
- Effectively communicate patient status updates to the clinical team to ensure continuity of care
Desired Qualifications:
- Active RN license
- Minimum one year of recent home health RN experience
- Proficient with OASIS documentation (required)
- Experience with HomeCare HomeBase (strongly preferred)
- Demonstrated ability to independently manage a full caseload and daily schedule of patient visits
- Reliable transportation and current drivers license
