Your next nursing role awaits: a 13-week full-time assignment where your expertise in home health can shine. We are seeking a skilled Registered Nurse with proven OASIS experience to deliver high-quality care in patients' homes, supported by a collaborative and professional care team.
Schedule:
- Full-time, Monday through Friday
- On-call responsibilities may be required
- Consistent daily visit expectations
Qualifications:
- Active RN license
- At least one year of recent experience as a Home Health RN
- OASIS documentation proficiency is essential
- Familiarity with HomeCare HomeBase is strongly preferred
- Ability to follow a set, full-time schedule and independently manage multiple daily patient visits
- Reliable transportation and a valid driver's license
Responsibilities:
- Deliver in-home skilled nursing care, including wound care, patient education, medication management, and disease monitoring
- Conduct timely, detailed patient assessments and maintain precise OASIS documentation
- Work closely with physicians and other healthcare professionals to create and adapt individualized care plans
- Clearly communicate changes in patient condition or care needs to the clinical team
- Ensure a compassionate and patient-focused approach in all interactions
