A dedicated and skilled Registered Nurse (RN) with strong OASIS proficiency is needed for an impactful 13-week home health contract. If you thrive on clinical excellence, organized documentation, and delivering compassionate carethis position allows you to make a meaningful contribution in a supportive environment.
Schedule:
- Full-time, Monday through Friday
- On-call duties may be required
- Structured daily visit expectations
- 13-week contract, with the potential for extension
Key Responsibilities:
- Deliver high-quality in-home nursing care and skilled interventions
- Conduct thorough patient assessments and ensure OASIS documentation is completed accurately and promptly
- Collaborate closely with physicians and multidisciplinary care teams to create and implement individualized care plans
- Provide wound care, manage medications, educate patients, and monitor chronic conditions
- Communicate significant updates or changes in patient health status to the clinical team
Required Qualifications:
- Active RN license
- At least 1 year of recent home health RN experience
- Proficiency with OASIS documentation is essential
- Experience with HomeCare HomeBase is strongly preferred
- Ability to efficiently handle a full-time schedule with daily home visits
- Valid drivers license and reliable transportation
