Job Description

Your next nursing opportunity starts here! Were looking for a dedicated Registered Nurse with strong home health and OASIS experience to step into a 13-week full-time contract. Make a direct impact delivering high-quality care to patients in their homes while utilizing your clinical skills and attention to detail.

Schedule & Structure:

  • 13-week contract, with possibility for extension
  • Full-time, Monday through Friday
  • Daily structured visits
  • On-call may be required

Key Responsibilities:

  • Deliver skilled, compassionate nursing care to patients in their homes
  • Complete comprehensive patient assessments and submit OASIS documentation accurately and on time
  • Develop and implement individualized care plans in collaboration with physicians and the greater care team
  • Perform wound care, patient education, medication management, and disease process monitoring
  • Communicate condition changes and updates to the clinical team promptly

Qualifications:

  • Active RN license
  • At least 1 year of recent home health RN experience required
  • OASIS experience required
  • Proficiency in HomeCare HomeBase is strongly preferred
  • Demonstrated ability to maintain a structured, full-time schedule with scheduled daily visits
  • Reliable transportation and a valid drivers license
