We are seeking a skilled Registered Nurse with recent home health experience and strong OASIS expertise for a 13-week full-time contract in home health care. This role is ideal for RNs who excel in clinical practice, detailed documentation, and maintaining a reliable routine.
Schedule:
- 13-week contract
- Full-time, Monday through Friday
- On-call duties may be required
Key Responsibilities:
- Deliver compassionate in-home nursing care and skilled interventions to patients
- Complete comprehensive patient assessments and OASIS documentation accurately and timely
- Collaborate with physicians and interdisciplinary teams to tailor individualized care plans
- Perform essential functions such as wound care, medication management, disease monitoring, and patient education
- Clearly communicate any updates or changes in patient condition to the clinical team
Qualifications:
- Active and unrestricted RN license
- At least 1 year of recent experience as a home health RN
- Proficient with OASIS documentation (required)
- Familiarity with HomeCare HomeBase strongly preferred
- Ability to manage a structured, full-time schedule
- Valid driver's license and dependable transportation
