Expand your practice and make a meaningful impact providing skilled care as a Registered Nurse in a dynamic home health setting. This field-based, full-time, 13-week contract position offers the chance to work independently with a supportive team, delivering vital care directly to patients homes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive assessments using OASIS and develop individualized care plans for each patient
• Deliver skilled nursing interventions such as wound care, IV therapy, medication administration, and chronic disease management
• Monitor and evaluate patient progress, recognizing and reporting changes in condition to interdisciplinary team members and physicians
• Educate patients and their families on disease management, medications, and effective preventive care strategies
• Document care accurately and promptly using Home Care Home Base (HCHB) electronic system
• Collaborate closely with multi-disciplinary team membersPT, OT, medical social workers, and home health aidesto coordinate patient care
• Take part in case conferences, staff meetings, and required continuing education
• Ensure compliance with documentation, agency, and regulatory requirements

Qualifications:

• Active Registered Nurse (RN) license in good standing for the state
• At least one year of recent home health experience, preferably in the field
• Demonstrated proficiency with Home Care Home Base (HCHB)
• Experience completing OASIS assessments
• Reliable transportation, valid drivers license, and active auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
