Your next nursing opportunity begins here! We are seeking a skilled Registered Nurse (RN) with a strong background in home health and proven OASIS experience for a 13-week full-time contract. Schedule:
- Full-time, Monday through Friday
- On-call responsibilities may be required
- Structured daily visit expectations
Key Responsibilities:
- Deliver skilled nursing care to patients in-home, ensuring clinical excellence and compassion
- Conduct thorough patient assessments, completing OASIS documentation accurately and promptly
- Collaborate closely with physicians and interdisciplinary teams to implement individualized care plans
- Provide wound care, medication management, patient education, and monitor chronic conditions
- Communicate patient status updates and any changes to the clinical leadership team
Qualifications:
- Active RN license required
- At least 1 year of recent RN home health experience
- Prior OASIS assessment and documentation competency
- Familiarity with HomeCare HomeBase (strongly preferred)
- Excellent organizational and time-management skills for a structured patient visit schedule
- Reliable transportation and a valid drivers license
