An experienced Home Health Registered Nurse (RN) is sought to deliver skilled nursing care to patients in their residences, ensuring high-quality, patient-centered services. This role demands an organized, self-driven professional proficient in clinical care and patient communication, capable of working independently in a field-based setting.
Responsibilities include conducting thorough OASIS assessments, developing personalized care plans, managing wound care, administering medications and IV therapies, and overseeing chronic disease management. The nurse will monitor patient progress, communicate changes to the healthcare team, provide patient and family education, and maintain accurate documentation using the Home Care Home Base (HCHB) system. Collaboration with therapists, social workers, and aides is integral to coordinated care delivery. Compliance with all regulatory standards and participation in continuing education and case conferences are expected.
This is a full-time, 13-week contract position scheduled Monday through Friday, from 8:00 AM to 5:00 PM, with a caseload ranging from 2532 points weekly (approximately 35 visits daily). Daily travel to patient homes within the assigned territory is required, with mileage reimbursement provided.
Qualifications:
- Active RN license in good standing within the state of practice
- Minimum of 1 year recent Home Health experience; field experience preferred
- Proficiency with Home Care Home Base (HCHB) and OASIS assessment tool
- Strong clinical assessment, critical thinking, and communication skills
- Valid drivers license, reliable transportation, and auto insurance
- Ability to manage a flexible, independent, field-based schedule
Preferred:
- BSN degree
- Experience managing a weekly caseload of 2532 points
- Familiarity with Medicare and home health compliance standards
