Key Qualifications:
- Active RN license
- Home Health experience preferred
- Strong collaboration and communication skills
- Ability to coordinate complex care plans with multiple disciplines
- Experience working with diverse patient populations, including chronic and high-utilization patients
Primary Responsibilities:
- Collaborate in developing, implementing, revising, and reporting on the case management program
- Serve as a liaison between patients, families, physicians, and the care team to ensure optimal outcomes
- Monitor appropriateness and timeliness of care, coordinating with other care team disciplines
- Maintain and review interdisciplinary care plans and manage resource utilization discussions with physicians
- Focus on complex cases, frequent emergency department users, chronic pain/substance abuse, and homeless patients
- Coordinate patient transfers to tertiary centers, ensuring continuity of information and care
- Oversee regulatory and accreditation data collection and compliance
