Job Description
Deliver high-quality care as a Registered Nurse specializing in Home Health, supporting patients in Honolulu, HI. This contract position offers the opportunity to work closely with a dedicated healthcare team to coordinate and optimize patient outcomes in a home care setting.
Key Responsibilities:
- Collaborate with patients, families, physicians, nurses, and multidisciplinary teams to develop and implement individualized care plans.
- Act as a key liaison ensuring effective communication among healthcare benefactors and the care team.
- Lead coordination of patient care activities, monitoring timeliness and appropriateness across disciplines.
- Oversee interdisciplinary care plans aligned with patients clinical needs and covered services, including diagnostic testing and treatments.
- Consult with physicians regarding resource utilization, treatment plans, discharge planning, and length of stay.
- Focus on complex patient cases including chronic pain, substance abuse, frequent emergency department use, and homelessness.
- Coordinate patient transfers to tertiary centers, ensuring seamless continuity of care.
- Collect and oversee data for regulatory and accreditation compliance.
- Manage high-utilizer patients through in-depth admissions review and care coordination.
Qualifications:
- Current Registered Nurse (RN) license with home health nursing experience.
- Strong clinical judgment and communication skills.
- Experience working with complex patient populations and multidisciplinary teams.
- Ability to manage multiple priorities and tailor care strategies for diverse patient needs.
