Your Next Nursing Opportunity Starts Herededicated professionals are needed for a full-time, 13-week home health nursing contract. Schedule:
- Full-time, Monday through Friday
- On-call responsibilities may be required
- Structured daily visit expectations
Key Responsibilities:
- Deliver high-quality in-home nursing care and skilled interventions
- Perform comprehensive patient assessments, ensuring OASIS documentation is accurate and promptly completed
- Collaborate closely with physicians and multidisciplinary care teams to support individualized care plans
- Carry out wound care, medication administration, patient education, and chronic disease monitoring
- Provide timely updates on changes in patient condition to the clinical team
Qualifications:
- Active RN license
- At least one year of recent home health nursing experience
- OASIS documentation experience required
- Proficiency with HomeCare HomeBase strongly preferred
- Ability to effectively manage a full-time schedule with daily visits
- Reliable transportation and a valid drivers license
