We are seeking a dedicated Registered Nurse (RN) with a strong background in home health and OASIS documentation for a 13-week, full-time contract assignment. This role is ideal for a skilled clinician who values autonomy while working collaboratively with a supportive care team.

Schedule and Hours:

• 13-week contract, full-time
• Monday through Friday schedule (on-call may be required)
• Structured daily visit expectations

Key Responsibilities:

• Deliver in-home nursing care and skilled interventions to patients
• Complete thorough patient assessments and ensure accurate, timely OASIS documentation
• Collaborate with physicians and interdisciplinary teams to create and follow individualized care plans
• Perform wound care, medication management, patient education, and monitor chronic conditions
• Maintain clear communication regarding patient status and changes with the clinical team

Qualifications:

• Active RN license
• Minimum one year of recent RN home health experience
• Proficiency with OASIS assessments and documentation
• Experience with HomeCare HomeBase (strongly preferred)
• Reliable transportation and a valid drivers license
• Capable of managing a structured, full-time schedule with multiple daily patient visits
