A dynamic home health opportunity is available for an experienced Registered Nurse seeking a full-time contract position. This 13-week assignment focuses on delivering exceptional patient-centered care in a field-based setting, with a structured weekday schedule that promotes both patient impact and work-life balance.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Role Overview:

• Manage a caseload averaging 30 points per week, typically 35 patient visits daily within an assigned territory
• Utilize the Home Care Home Base (HCHB) system for documentation
• Conduct comprehensive OASIS assessments and develop tailored nursing care plans
• Provide skilled care such as wound management, IV therapy, medication administration, and chronic disease support
• Consistently monitor patient status, communicating promptly with physicians and the interdisciplinary care team
• Engage patients and families in education around care processes, medications, and health maintenance
• Ensure all documentation and care delivery aligns with regulatory and agency requirements
• Collaborate closely with therapy, social work, and home health aide teams for holistic, coordinated care
• Participate in ongoing case conferences, staff meetings, and relevant continuing education sessions
• Daily travel within your care area, with mileage reimbursement

Desired Qualifications:

• Current, active RN license in good standing for the state of practice
• Minimum of one year recent, hands-on home health RN experience (field-based strongly preferred)
• Solid proficiency with both Home Care Home Base (HCHB) and OASIS required
• Reliable transportation, valid drivers license, and appropriate auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
