We are seeking a dedicated Registered Nurse with demonstrated expertise in home health care and OASIS documentation for a full-time, 13-week contract assignment. Schedule:
- Full-time, Monday through Friday
- Possibility of on-call assignments
- Structured daily visit expectations
Key Responsibilities:
- Deliver skilled nursing care and clinical interventions in patients homes
- Complete thorough patient assessments and document using OASIS accurately and timely
- Collaborate with physicians, care coordinators, and ancillary teams to develop and implement individualized care plans
- Perform wound care, medication administration, patient and family education, and conduct disease process monitoring
- Consistently communicate any changes in patient status to the interdisciplinary team
Qualifications:
- Current, active RN license
- Minimum of one year recent home health RN experience
- OASIS documentation experience required
- Proficiency with HomeCare HomeBase is strongly preferred
- Comfortable managing a full-time caseload with daily visits
- Reliable transportation and valid drivers license

$2,193 - $2,312/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
