Your next nursing opportunity starts here. We are seeking a dedicated Registered Nurse (RN) with OASIS expertise to support patients through a 13-week full-time home health contract. Schedule:
- 13-week contract, with the possibility of extension
- Full-time, Monday through Friday
- On-call participation may be required
- Daily visit quotas expected
Key Responsibilities:
- Deliver comprehensive in-home nursing care and skilled clinical interventions
- Assess patients and complete timely, accurate OASIS documentation
- Work closely with physicians and interdisciplinary teams to create individualized care plans
- Perform wound care, manage medications, provide patient/family education, and monitor disease progression
- Maintain clear communication with the clinical team regarding patient updates and any changes in condition
Qualifications:
- Active RN license
- At least one year of recent home health RN experience
- OASIS documentation proficiency
- HomeCare HomeBase experience strongly preferred
- Demonstrated ability to manage a full caseload and structured daily schedule
- Valid drivers license and reliable personal transportation
