Schedule/Hours:

• Monday to Friday, 8:00 AM 5:00 PM
• Day shifts (8 hours each)
• 8-week contract, with potential extension up to 12 weeks depending on census

Qualifications:

• Active RN license
• Prior experience in Home Health and HCHB systems, plus travel nursing experience
• At least 1 year of recent case management experience required
• Valid drivers license and reliable vehicle
• Ability to travel daily throughout designated areas

Responsibilities:

• Deliver skilled nursing care to patients in a home health setting
• Manage a caseload, including comprehensive assessments, care planning, and coordination
• Provide patient and family education on disease management and self-care techniques
• Collaborate with interdisciplinary healthcare teams to ensure continuity of care
• Maintain accurate documentation in compliance with regulatory guidelines
• Travel throughout assigned territory to meet patient needs (mileage reimbursement provided at the federal rate)
