Seeking an experienced and compassionate Registered Nurse to provide home health care in a dynamic, patient-focused environment. This contract role covers an 8-week period, with the potential to extend up to 12 weeks based on census needs. The assignment entails traveling throughout the Greensboro and High Point area during weekdays, delivering vital home health services to individuals in their residences.

Schedule:

• Monday to Friday: 8:00 a.m. 5:00 p.m.
• Five 8-hour day shifts per week

Key Qualifications:

• Valid RN license required
• Minimum of 1 year experience as a case manager in home health setting
• Experience with Home Health Case Management (HCHB) and travel assignments required
• Own a reliable vehicle and possess a current drivers license
• Ability to manage a patient caseload independently and organize daily travel routes
• Strong clinical, communication, and critical thinking skills

Primary Responsibilities:

• Conduct in-home visits to assess, plan, implement, and evaluate individualized plans of care
• Manage a dynamic caseload, coordinating services with other clinicians as needed
• Document care in HomeCare HomeBase (HCHB) or similar EMR systems
• Educate patients and families on treatment, prevention, and self-care strategies to support positive outcomes
• Serve as the primary point of contact for assigned patients, ensuring a seamless continuum of care throughout the home health journey
