A rewarding opportunity is available for a Registered Nurse with home health and travel experience to support patient care in the Greensboro/High Point area. This role requires an RN with the ability to manage a caseload independently, offering in-home care coordination and support for patients across the region. The assignment is a contract lasting 8 weeks, with the potential to extend to 12 weeks based on census needs.

Responsibilities:
- Manage and coordinate home health nursing cases throughout the Greensboro and High Point areas
- Provide direct patient care and ensure compliance with care plans
- Travel independently between patient homes using your own vehicle
- Collaborate with multidisciplinary teams to optimize patient outcomes
- Document patient visits and case management activities thoroughly and timely
Qualifications:
- Active RN licensure with home health nursing experience
- Minimum of 1 year case management experience required
- Comfortable with frequent travel during work hours
- Ability to manage multiple cases and prioritize effectively
- Valid drivers license and reliable transportation necessary
Schedule/Hours:
- Monday through Friday
- 8:00 AM to 5:00 PM (day shift, 5 days per week)
Additional Information:
- Mileage reimbursement provided at the federal rate
- This is a contract position with emphasis on home health nursing in the outpatient setting
