Job Description
Provide skilled nursing care in patients homes across the Greensboro and High Point areas, focusing on comprehensive case management for a diverse patient population. This role involves coordinating and managing multiple cases independently while delivering high-quality home health care services.
Responsibilities:
- Manage a caseload of patients requiring home health nursing assessments and treatments
- Coordinate care plans with interdisciplinary teams and communicate regularly with patients, families, and providers
- Travel within the Greensboro and High Point regions to provide in-home nursing care
- Document patient progress and maintain compliance with regulatory standards
- Provide patient education regarding treatments and self-care techniques
Qualifications & Desired Experience:
- Current Registered Nurse (RN) licensure in the state of North Carolina
- Minimum of 1 year of case management experience in home health nursing
- Experience with travel nursing or home health nursing preferred
- Reliable transportation and willingness to travel throughout the day is essential
- Strong organizational, communication, and patient care skills
Schedule & Hours:
- Monday through Friday, 8:00 AM to 5:00 PM (5 days, 8 hours per day)
