Experience the rewarding impact of providing essential care to patients in their homes as a Registered Nurse specializing in Home Health. This contract opportunity involves supporting individuals in the Greensboro/High Point area with a focus on high-quality, compassionate care.
Schedule/Hours:
- Monday to Friday, 8:00 AM 5:00 PM
- Day shifts (8 hours each)
- 8-week contract, with potential extension up to 12 weeks depending on census
Qualifications:
- Active RN license
- Prior experience in Home Health and HCHB systems, plus travel nursing experience
- At least 1 year of recent case management experience required
- Valid drivers license and reliable vehicle
- Ability to travel daily throughout designated areas
Responsibilities:
- Deliver skilled nursing care to patients in a home health setting
- Manage a caseload, including comprehensive assessments, care planning, and coordination
- Provide patient and family education on disease management and self-care techniques
- Collaborate with interdisciplinary healthcare teams to ensure continuity of care
- Maintain accurate documentation in compliance with regulatory guidelines
- Travel throughout assigned territory to meet patient needs (mileage reimbursement provided at the federal rate)
