This 13-week full-time contract in Hernando, MS, offers a chance to deliver compassionate, skilled nursing care to patients in their homes while working within a structured schedule Monday through Friday.
Key Responsibilities:
- Provide comprehensive in-home nursing care and skilled interventions.
- Conduct patient assessments and ensure timely and accurate OASIS documentation.
- Collaborate with physicians and interdisciplinary care teams to develop and implement individualized care plans.
- Perform wound care, medication management, patient education, and chronic disease monitoring.
- Communicate patient condition updates effectively to the clinical team.
Qualifications and Experience:
- Active RN license.
- Minimum of one year of recent home health nursing experience.
- Required proficiency with OASIS documentation.
- Familiarity with HomeCare HomeBase software is strongly preferred.
- Ability to manage a structured, full-time schedule with daily patient visits.
- Reliable transportation and valid drivers license.
