Your next nursing opportunity starts here. An experienced Registered Nurse with OASIS expertise is needed for a 13-week, full-time home health contract. This position requires a confident professional skilled in clinical care, meticulous documentation, and managing a set scheduleproviding the chance to make a meaningful difference in patients lives.
Schedule:
- 13-week contract, with the possibility for extension
- Full-time hours, Monday through Friday
- On-call duties may be required
Qualifications:
- Active RN license
- At least 1 year of recent home health RN experience
- OASIS assessment and documentation proficiency
- HomeCare HomeBase experience strongly preferred
- Reliable transportation and a valid drivers license
- Comfortable working within structured daily visit expectations
Key Responsibilities:
- Deliver in-home skilled nursing care, assessments, and interventions
- Complete OASIS documentation promptly and accurately
- Partner with physicians and interdisciplinary care teams to design and implement care plans
- Perform wound care, medication management, patient and family education, and disease progression monitoring
- Communicate patient status changes or concerns to the appropriate clinical team members
