Your next nursing opportunity is here! A rewarding full-time contract is open for a Registered Nurse with strong home health experience and a passion for delivering exceptional patient care. This 13-week assignment offers the chance to directly impact the lives of patients within the comfort of their homes, utilizing your clinical knowledge, assessment skills, and expertise with OASIS documentation.
Schedule:
- Full-time, Monday through Friday
- On-call responsibilities as needed
What to Expect:
- 13-week contract with the possibility for extension
- Structured daily visit schedule for effective patient management
Key Responsibilities:
- Deliver in-home nursing care, including skilled interventions and health assessments
- Accurately complete OASIS documentation in a timely manner
- Work collaboratively with physicians and multidisciplinary teams to develop and implement individualized care plans
- Provide wound care, medication management, patient education, and disease monitoring
- Maintain clear and timely communication regarding patient status to the clinical team
Who You Are:
- Active, unencumbered RN license
- Minimum of one year recent home health RN experience
- Proficient in OASIS documentation (required)
- Strongly preferred: proficiency with HomeCare HomeBase EMR
- Able to thrive within a full-time, structured home health visit schedule
- Dependable transportation and a valid drivers license
