Your next nursing opportunity starts here. An experienced Registered Nurse with OASIS expertise is needed for a 13-week, full-time home health contract. This position requires a confident professional skilled in clinical care, meticulous documentation, and managing a set scheduleproviding the chance to make a meaningful difference in patients lives.

Schedule:

  • 13-week contract, with the possibility for extension
  • Full-time hours, Monday through Friday
  • On-call duties may be required

Qualifications:

  • Active RN license
  • At least 1 year of recent home health RN experience
  • OASIS assessment and documentation proficiency
  • HomeCare HomeBase experience strongly preferred
  • Reliable transportation and a valid drivers license
  • Comfortable working within structured daily visit expectations

Key Responsibilities:

  • Deliver in-home skilled nursing care, assessments, and interventions
  • Complete OASIS documentation promptly and accurately
  • Partner with physicians and interdisciplinary care teams to design and implement care plans
  • Perform wound care, medication management, patient and family education, and disease progression monitoring
  • Communicate patient status changes or concerns to the appropriate clinical team members
