We are seeking a skilled professional who excels in patient care, meticulous OASIS documentation, and thrives in a dynamic, full-time schedule. Schedule/Hours:
- Full-time, Monday through Friday
- On-call responsibilities are likely as part of the weekly schedule
Key Responsibilities:
- Deliver comprehensive in-home nursing care and clinical interventions
- Conduct patient assessments; complete OASIS documents accurately and on time
- Collaborate with physicians and interdisciplinary teams to craft personalized care plans
- Perform wound care, medication management, patient education, and disease monitoring
- Maintain prompt and effective communication with the clinical team regarding patient status
Qualifications:
- Active, valid RN license
- At least 1 year of recent home health RN experience
- Proficient with OASIS documentation (required)
- Proficiency in HomeCare HomeBase is strongly preferred
- Reliable transportation and a valid drivers license
- Ability to efficiently manage a structured schedule of daily patient visits
