An opportunity is available for an experienced Registered Nurse (RN) specializing in home health care to provide compassionate, comprehensive nursing care on a contract basis. This role is ideal for a dedicated professional with a strong clinical background, particularly in OASIS documentation, who is committed to improving patient outcomes in the comfort of their own homes.
Key responsibilities include:
- Delivering in-home nursing assessments and ongoing patient care, including vital signs monitoring, medication management, and wound care.
- Creating and implementing individualized care plans tailored to each patients unique needs.
- Educating patients and their families on disease management, treatment protocols, and preventive health strategies.
- Coordinating with physicians, therapists, and healthcare professionals to ensure integrated and effective patient care.
- Accurately documenting all patient interactions and clinical information in electronic medical records (EMR).
- Observing and reporting any changes in patient condition to the supervising physician.
- Maintaining compliance with all applicable state and federal home health regulations.
Desired qualifications and experience:
- Active Registered Nurse (RN) license.
- Minimum of three months home health nursing experience, with proven proficiency in OASIS.
- Experience with Home Care Home Base (HCHB) software is highly advantageous.
- Demonstrated clinical skills including IV therapy, wound care, and medication administration.
- Strong proficiency with EMR systems.
- Excellent communication and interpersonal abilities.
- Self-motivated with effective time management skills.
- Valid drivers license and reliable transportation required.
