Your next nursing opportunity starts here. We're seeking a dedicated Registered Nurse with proven home health and OASIS experience for an impactful 13-week full-time contract. This position offers the chance to make a difference by delivering patient-centered care in a home setting, while showcasing your clinical skills and attention to precise documentation.
Schedule:
- 13-week contract duration, with potential for extension
- Full-time, Monday through Friday
- On-call responsibilities may be required
Key Responsibilities:
- Deliver in-home nursing care, including skilled interventions tailored to each patient's needs
- Conduct comprehensive assessments and complete all OASIS documentation promptly and accurately
- Collaborate with physicians and the care team to implement individualized care plans
- Perform wound care, medication administration, patient education, and monitor chronic conditions
- Communicate any changes in patient status or care needs effectively with the clinical team
Qualifications:
- Current and active RN license
- At least 1 year of recent home health RN experience
- OASIS documentation expertise is required
- Strong proficiency with HomeCare HomeBase (preferred)
- Consistent ability to manage a structured schedule and daily visit expectations
- Reliable transportation and a valid drivers license are mandatory
