This 13-week, full-time contract requires a compassionate RN ready to provide top-tier patient care and expertly manage clinical documentation. Schedule:
- Full-time, Monday through Friday
- On-call responsibilities may be included
Key Responsibilities:
- Deliver in-home nursing care and skilled clinical interventions.
- Conduct thorough patient assessments and ensure timely, accurate OASIS documentation.
- Collaborate with physicians and multidisciplinary care teams to implement customized care plans.
- Perform wound management, medication administration, patient education, and ongoing disease monitoring.
- Maintain clear communication with the clinical team regarding patient status and updates.
What Were Looking For:
- Current, unencumbered RN license.
- At least 1 year of recent home health nursing experience.
- OASIS documentation expertise (mandatory).
- Proficiency with HomeCare HomeBase software is strongly preferred.
- Consistent ability to follow a structured schedule handling daily patient visits.
- Reliable transportation and a valid drivers license.
