Schedule/Hours:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM

Responsibilities:

• Conduct comprehensive OASIS assessments and craft individualized care plans for patients in their homes
• Deliver skilled nursing care including wound care, IV therapy, medication administration, and chronic disease management
• Monitor patient progress, promptly communicate changes in condition, and coordinate care with physicians and team members
• Educate patients and families regarding disease processes, medications, and preventive health measures
• Accurately document all care activities using Home Care Home Base (HCHB)
• Collaborate with interdisciplinary teams including therapists and social workers to ensure holistic, seamless care
• Regularly attend case conferences and participate in ongoing professional development
• Maintain compliance with regulatory and agency standards

Qualifications:

• Active RN license in Pennsylvania (in good standing)
• At least one year of recent home health nursing experience, with preference for field-based work
• Proficient with OASIS and the HCHB electronic documentation system
• Reliable transportation, current drivers license, and up-to-date auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
